Welcome! Thank you for choosing Modern Dermatology Atlanta for your skin care consultation. Please complete the form below and select a time that best serves your needs. 

NEW PATIENTS:

You will receive a registration email and phone call between 24-48 hours after you request an appointment. Please follow the link to activate your "Patient Portal" and complete our electronic 'Check-In.'  Please note that we CANNOT confirm your appointment until this step is completed and Insurance information is submitted. In addition, a member of our staff may reach out to you to confirm the details of your request.

ESTABLISHED PATIENTS:  

You will receive a link in your Patient Portal that will allow you to check in online the day of your visit. If you Patient Portal is inactive, please call us to obtain a new activation link
