Appointment Request

To directly schedule an appointment from our website, please fill out the form below. 

*PLEASE READ PRIOR TO SCHEDULING*:

₁₎ Select  NEW PATIENT or EST PT from the Type of Visit dropdown menu.

2) In the "Reason for Visit" box, please type the single main concern you'd like to address at this appointment.

3) Please do not request surgical appointments online. You can reach us by phone at (770) 250-7199 (ext. 2).

4) Appointments are NOT confirmed until you have spoken with a member of our staff.

5) Appointments available through online booking may be with Naga Meduri, M.D. or her Physician Assistant, Madeline Shedd.  The provider will be confirmed with you when you speak with a member of our staff.

NEW PATIENTS:

You will receive a registration email and phone call between 24-48 hours after you request an appointment. Please follow the link to activate your "Patient Portal" and complete our electronic 'Check-In.'  Please note that we CANNOT confirm your appointment until this step is completed and Insurance information is submitted. In addition, a member of our staff may reach out to you to confirm the details of your request.

ESTABLISHED PATIENTS:  

Please follow the link to activate your "Patient Portal" if you do not already have one.  If you already have a Patient Portal, you will receive a link to check in online the day of your visit.
